Tax Receipt Request

 

Thank you for your support of the Avon Walk for Breast Cancer.  If a donation was made online, a receipt was issued via email at the time of the donation.  For donations of $250.00 or more that were made "offline," receipts will be issued by the Avon Foundation at the end of January.  For lesser amounts, a cancelled check or credit card bill is sufficient under current tax law.

If an email receipt was misplaced for an online donation, please include the details below so we may issue a replacement receipt. Include the name of the donor, name of Walker supported, clarification of which Avon Walk city (Boston, Charlotte, Chicago, DC, Houston, LA, NY, Rocky Mountains, San Francisco), the donation amount and date or approximate date of donation.
